I really love to draw pirate flags, most of the pirate groups here in me country use pirate flags created by me. I think the game could gave us the feature to upload or draw a pirate flag to use.
Why? Reputation. Real pirates used flags and become famous on the seas. Using our own flags could be fun, been recognized even on far distances, i really think that's cool.
This discussion has been around for quite a while. Making a customisable flag can be a challenge to implement in the game. Especially a hand drawn one.
I think the easiest implementation would be to allow a player to make a flag with a few presets, let's say:
And that should be about it. I can go on and on about this subject, but there are lot of games that already implement a feature like this.
@targasbr custom flags would be cool, but allowing people to upload their own really opens up a whole other can of worms.
I'm not sure which installment it was, I think it was Black Ops 2, but one of the Call of Duty games had a real big problem with swastikas showing up all over the place. I'm all for custom, but within limits I suppose.

I don't remember where, but there is a game where all upload images pass on a previous approval, and its made by the community. On xbox profile picture we use that. I think it's a bot, dunno.

I really loved your idea, but i think it's has lotta limits...
So... There is a game called Top Gear Rally (N64), and in this game we can easily customize our car. I didn't find a video, but there is a screenshot. There is a "paintbrush" inside the game. Simple, but a good artist could do lotta good things.
It's simply not that easy, @targasbr. If we want custom flags, our request has to start out simple. There has to be a lot of limits.
It would have to be like Forza or Need For Speed visual customization where you can choose from a set of select images and shapes and combine them to create what you would like. This would help eliminate offensive pictures from being uploaded as flags. Otherwise Rare would need to screen every flag that was uploaded.
