Forsaken ashes eye of reach glitch

Is any one else finding that the textures for the forsaken ashes eye of reach are really glitched, also when ever I have it equip and I try to swap weapons my game has a memory spike and crashed, this may be due to me playing on Xbox but I was just curious if anyone else was having the same issue.
