I've done hundreds of forts and never seen that happen.
@wolfmanbush me too. Had around 1y break from SoT and today started to play it again.. and it happened me solo doing my first fort.
Well.. my question is answered. Hope devs gonna look at it and if needed - fix. :)

I did a fort of the dammed yesterday (duo Brig) and the very same thing happened to me.
I have also never seen it happen before so I emptied the room and then took the key to go get another Ritual skull to open the room again.
I noticed that I showed up on the map to other players with a red/white /black skull logo that other players on the map had not seen before and they all gave chase not because they new I had a key but because none of them had seen the logo before and they wondered who the hell I was!
Got another Skull went back and opened another room.
@keslv When you server merge the vault opens in case the key was left at the fort (and therefore is left on the other server.) I assume you brought the key back to your ship which means bonus key for you.
@jbinkc well, that makes sense
after selling all my loot didnt see any fort up, so i just droped it in the sea.. (:
