So i did some of the vaults, failed with one even though the fourth combination was right it still diden't open.
This was the first one from GOLD HOARDER that i did, the others has been from Larissa and they worked FINE.
It was really odd, it happend on Krakens Fall (completed one there before) and the combination was Chest Key&lock two chains = The fourth was all of them combined but still it diden't work.

Im well aware of this and i tested them all, and i got the same combination later on Devils Ridge and that worked just fine.
So this was just a brutal bug, my friend tested as well and it still diden't work.
Im a well experianced player so i know for a fact that i had the right combination as i did it a thousand times on insider. (and diden't fail a single time)

Im blaming myself for being mad about it and then got brutally uglyworded by a doublegunning gally which bunnyjumped all over the place. (They need to nerf that stupid jumping) - When i should've checked my horizons better.
But the vault thing was rather BS from the games side

Are you sure you didnt actually have them mirrored? Cause some of the pictures (like lock and chains) have mirrored versions of them and when you're doing it fast you might think you have it correct but it's actually mirrored.
If you're 100% sure you had it right maybe it was a bug (the vault worked for me) and could possible be fixed with todays patch.
I've done 3/3 so far.
The thing to notice is that if gives you 3 pictures with 4 dials to turn.
Dials either go 1-3 or 2-4. Just have to really pay attention to the orientation of the pictures. Like @D3ADST1CK said, the pictures look the same but they aren't.
Some of the horizontal chains look the same but if you look carefully they tilt at different angles. Key and lock are also different.
You said it was the gold hoarder quest?? If I get a chance to try it out before they patch it, I'll let you know what I experience.

I did these on insider alot...so im well aware :P
