I think that it would be really neat to have the ability to duel someone. More specifically... duel a friend. I think it would be neat if you could fight your crew to have fun or practice PvP. I think it could be done like Terraria, where everyone has to have PvP on to actually fight. I also think that it would be really cool to have a new voyage where its a raid on a player fort. Where players man the cannons and defend the fort from naval fire.
Organized PvP outside of ship-to-ship combat.
@sp1kes05 I can't say much about wanting more TDM aspects, but they are introducing a Fort where you'll have to defend it from NPC ships that try to take you out. The Skull Forts though, not the Sea Forts. Maybe you'll enjoy that content when it hits?
Custom servers will provide this. I know I plan on getting a custom server and doing exactly this among other things.
