So volcano hit me with a rock, a fire starts, i extinguish it with the texture of it going away, but the ship is still burning and holes appear even though i sailed away from the volcano and am not near anything. I sink because i die of the fire. Is it a bug or did i miss something?

You missed something. Sometimes if there is a black smoke in an area of the ship, that means the ship will catch one fire soon. Potentially, you can put out all fires, but if a black smoke pops up, it will shortly catch the ship on fire again.
Just like with a regular fire, you can put out the smoke with water.
Another reason is that there might be another fire on the ship that you haven't noticed or haven't put out, if you let it burn, it will create more black smoke around the ship, eventually becoming flames. Too many flames on a ship will cause holes to open up on thee ship, eventually sinking you if you continue to ignore it or don't take car of it.
