With the PL exclusive cosmetics, do you only earn them when you reach said rank and you are a PL? Say I am not a PL and get to lvl 10 which has a PL exclusive item. When I reach lvl 20 and become a PL, do I learn the item from lvl 10?

Yes. If you are not PL when you get to a PL reward teir, the previous teirs (only from that season) unlock.
At least they're supposed to. SoT after all lol.
