has any encountered an issue with both vaults being double doored? we just opened them both and another door appeared. we were not able to access the loot from the fortress !

Aye, it happened to me today. I had opened the small jail and the one on the top and got the chests out of both of them. Then I killed the first boss and opened the large room (got most of the chests out).
When I killed the jailer I wanted to open the jail in the larger room, I did but when I tried to walk in there was a door in front of me and next to me. The door of the small jail was also closed and open (so I couldn't get the gold piles).I did not server merge - a frequent cause for issues, but I did got killed on the fort - but not after I got the Jailor's key.
I created a ticket (https://support.seaofthieves.com/requests/new), I suggest you do the same, so they know it isn't a one-off but happens more often.
Who knows, they might fix it (doubt it over the weekend and right before a new Season) or give an extra voyage.
