Horizontal like it is and was demonstrated in the teaser, you need to consider l/R barrel when you're aiming. That skill thing the pvp pros like to caw about. Aligned vertical, that balance bit is tossed overboard for center mass aiming.

Over and under design is something that may happen when the different cosmetic variants are released. We have an EoR that looks like a crossbow and cutlasses that use rapier skins so I'd say it's possible we may get this.

Kinda unrelated but if the throwing knife takes up a throwable slot, I'd like a pencil cosmetic variant. So that I can run around with those, a flintlock and a double-barrel flintlock like a pirate John Wick.

Im sure plenty of cosmetic variants will add this in the future, and if not, its a huge missed chance.

@pithyrumble the op is right, the shot goes down the middle no matter what barrel it comes out of.
