You need to play 1 hour a week for 15 weeks. They do not need to be consecutive. If you skip a week you just dont get credit. It's worth pointing out that sometimes the banner doesnt appear in insider and your rewards are still on the live servers.

Edit: reset is on friday...so if you played on saturday you have to wait until friday (midday I guess)
@schwammlgott reset is actually midnight Thursday into Friday on British time. Being an EST player I do my insider tests on Thursday night.
