I think that the fountain of youth is a great idea!
The fountain of youth should be a goal for players really wanting to "reset" their appearances. More than a mere customization tool you pay for; It should be part of who you are.
|Off Topic| Arguably your cash is your worth but I'd say :
"meh ... your appearance is your worth or displayed worth."
You can chose to display your worth by being ostentatious with titles and outfits, changing it every chance you get to show how much stuff you bought ...

I agree with the fountain of youth resetting your character to a younger age.

We currently have the Legend's achievement. It is merely a grind that can be optimized and has been. For many players have reached the Legends status by now. (Numbers would be cool.)
I would prefer the fountain of youth to be a great achievement won through hardship and perseverance.
that could be achieved in one single segment of playing. Not as short as a fort, but more like a long series of quests (5-6 maps ?? More ? Less ?).
|Trigger For Quest| Much like bottle found on beaches and at sea in shipwrecks, it would be a rare type of bottle that contains the "Fountain Of Youth" quest a.k.a Character's physical appearance reset /edit quest.
|Difficulty| The difficulty would have to be asserted when implemented based on content.
But currently I'd say defeat the equivalent of half a fort on each island and a full fort on the last. Creating different types of enemies wouldn't be necessary but encourage long term.
Need adjustments, but I think the hardest it is for us, even the more overpowered, the best! I would force us to have to cooperate with two or more crews to achieve. That sounds like a barrier to some but is gold to others.
|Reward| You get to reset your appearance to the a different brackets of age (2 currently ? I have seen old and young especially with the Betas and STs) and get a title.

You would get to refill your cup at the fountain of youth and drink from it.
Adding how much you drink could of determine the age of choice. Much like the alcohol we drink, it would affect not the game play but the appearance.
||Maybe| It should pose a threat to having your character completely deleted; should you abuse it for instance. Much like our state changes when we get drunk.|
Moving on to the reason why we should be looking forward to this on the
Short term period - rushed- .
Imagine the fun seeing your mate get younger and younger ... Even two sips would be fun in the current state of the game. And the thrills of such an adventure once in a while.
Long term - safe and polished -
Let's say aging is a thing in game. Having your character get old is fun but you might wanna keep all your life's achievements and booty. But merely want to look younger again and erase the traces of time on your basic raw skin you fell in love with at first sight.
So aging would be the first glimpse of traces of time on your body.
I would suggest that in order to have players really go for this feature once in while, those traces of time are necessary for an awesome immersion.
|Wonkyness| These could be affected by the number of deaths in the first place. At random you get either a scar or a missing tooth or and ear part missing etc ... every 100 deaths?
(Pushing this further we could assign each type of Wonkyness to a certain value. Headshots get you facial scares.
Ear parts missing for sharks.
Etc ...)

Eating too much bananas while being full health (over healing) could make you fatter. The changes would be determined on quotas. The closest to reality the better. Not eating ever and constantly dying without eating bananas would make you skinnier. Arguably ...
