I just finally sold the 360 villanious skulls.
As soon as I sold the last one the message of the commendation completion popped up but the xbox achievement message didn’t.
So I checked the souls order reputation screen to see if the commendation was actually completed and yes, it was “ticked”. Then I checked if I could equip the title but it wasn’t there.
Sp to sum up, I have the commendation completed but I don’t have the title neither the xbox achievement.

@asran-stark I believe the title has to be purchased from a vendor. It's not one of the titles that is given upon earning, just unlocked to buy.
As for the achievement, this is a common problem but not as bad as you think. The achievement progress usually slacks behind the commendation slightly. I'm willing to bet you only need a few more skulls for the achievement. The same will probably happen with the 1000 miles sailed, and 250 voyages completed commendations and achievements.
@testakleze you were absolutely right about the title, I had to buy it.
About the achievement, the xbox menu says I'm at the 98%, so maybe I need to sell 7-8 skulls more.

Some of the OG titles are needed to buy from the vendors when you have accuired them, usally tells you if it's in your chest or if you have to visit a shop.
The Xbox achievements sometimes struggles to count some items but usually isn't too bad. Like you said, you were on 98% which should just be a few more!
