Missing Seeker of Lost Souls Title

Hi, I've completed the commendation for buying 250 OoS voyages, and have recieved the commendation.

I even received an in-game prompt that I received a new title to my vanity chest.

I've been through my title list numerous times, one by one. However I do not see the seeker of lost souls title in my list anywhere.
If I'm not mistaken the icon for the title should look like this:

but there is no such icon in my title list.

The closest thing is the mercenary of the ancient order title but that is the only one there.

Here are my OoS stats.

and commendation completion to grade V.

I have the mercenary of the ancient order title, but not seeker of lost souls, so very odd.

I have server-hopped and relogged to try and resolve the issue.

Would obviously like the title so looking forward to getting this sorted, thanks.
