Glitches I've had so far.

I just started the first tale of the five new ones and I've already came across a not so good bug. I was going towards the portal when a meg attacked me. I just didn't pay much attention to it and went into the portal instead. Inside the portal/sea of the damned, the meg was actually above me and was trying to attack me again. Not only that but the tale and meg music were overlapping as well. Another glitch I came across is one I've had for a while now. This could be on my end but every once in a while, almost all of the textures in the game become smooth and blurry as if they haven't loaded in. Is this a common glitch or is it just my end?
