Do ancient skeletons have certain spawn locations? Or is it that they’re just more likely to show up in certain spots more than others but they can spawn anywhere? I’ve had ancient skeletons spawn at FOTD 2 times, but never saw them anywhere else.

No, it's totally random. They don't spawn on Outposts, though. On top of that, they're very, very rare.
