Can we please Have any icon on the screen that says our voice chat is on, I had an incident with it countless of times , thinking I'm muted but finding out that everyone can hear me

There is an option under Audio Settings to display a speech bubble (shows in the top right corner) when you are transmitting your voice in-game.
