But I didn't. Was solo for around 3 hours, made some great gold, and then "Captain has left the crew". Didn't alt-f4, or disconnect etc. Is this a known bug?

Yes, this is a known bug. I believe they have tried to patch it but it still happens occasionally. If I remember correctly, even though it says you left you still make progress on your milestones. Though that might also depend. I had the captain left bug only once but I was still making progress on my milestones.
